According to a notice posted on their website, the Concentra data breach was reported after they were affected by a recent cyber security attack at Perry Johnson & Associates, Inc. PJ&A is a medical transcription company that provides its services to Concentra. On May 2, 2023, PJthe company detected suspicious activity within its computer network prompting them to launch an investigation. It was confirmed that an unauthorized party was able to access the company’s computer network between March 27, 2023 and May 2, 2023. PJ&A learned that the unauthorized party was able to access Concentra patient information between April 7, 2023 and April 19, 2023. On February 4, 2024, Perry Johnson & Associates sent out data breach letters on behalf of Concentra to those individuals whose information was affected by the breach.
Concentra, Inc. is a healthcare services provider based out of Addison, Texas that is a division of Select Medical, which is a large nationwide healthcare company. Concentra itself operates a network 
of approximately 520 urgent care clinics in 44 states.
