The incident was discovered on January 2, 2025, when the Community Health Center detected suspicious activity within their computer network. An investigation revealed that unauthorized actors had gained access to the network and exfiltrated files containing confidential information. The compromised data includes names, dates of birth, addresses, phone numbers, email addresses, diagnoses, treatment details, test results, Social Security numbers, and health insurance information.
On January 30, 2025, Community Health Center sent out data breach letters to those individuals whose information was affected by the breach. As part of their response to the incident, Community Health Center is offering affected individuals 24 months of complimentary credit and CyberScan monitoring services.
